This is the mechanistic link between abdominal fat and insulin resistance, dyslipidemia, hypertension, and type 2 diabetes — the cluster of conditions collectively known as metabolic syndrome. How to Measure Waist and Hip Circumference Correctly

The World Health Organization's STEPS protocol defines the waist measurement point as the midpoint between the lower margin of the last palpable rib and the top of the iliac crest, the bony ridge of the pelvis — for most people this falls close to, but not exactly at, the navel. Measurements should be taken standing, with the tape held snug but not compressing the skin, parallel to the floor, at the end of a normal exhalation rather than while holding the stomach in. Hip circumference is measured separately, around the widest point of the buttocks. The single most common measurement error is substituting the visual location of the navel, or the top of a trouser waistband, for the true anatomical landmark — both tend to sit lower than the correct measurement point and can meaningfully skew the result.

WHO Risk Categories and Clinical Significance

Worked Example: Calculating and Interpreting a Real WHR

Consider a man who measures a 38-inch (96.5 cm) waist circumference and a 40-inch (101.6 cm) hip circumference, following the WHO measurement protocol described above. His waist-to-hip ratio is 38 ÷ 40 = 0.95, or equivalently 96.5 ÷ 101.6 = 0.95 in metric units — confirming that the ratio is unaffected by which unit system is used, provided both measurements share the same unit.
